PRIVATE TRANSPORTATION RELEASE CONSENT FORM

At times it becomes necessary to use private vehicles to transport students to and

from Yawa Inc. activities. When this occurs, Yawa Inc. requires that the student sign
the Private Transportation Release Consent Form that appears below:
By signing this form, I hereby release Yawa Inc., as well as its directors, officers,
administrators, employees, or other agents from all liability or damages for any and
all injuries arising from the negligence of any of the above while traveling to this
activity via private transportation.
