I, PGI FRANCIS LOUIE A. HUMAWID of Class MATATAG Batch 3 2013, presently
assigned in this prison respectfully would like to request from your good office to change my
item from PRISON GUARD I to Nurse I. That I am a graduate of Bachelor of Science in
Nursing at SOUTHWESTERN UNIVERSITY CEBU, CITY in the year 2008 and successfully
passed the Nursing Board Examination of the same year. I have been employed at Abuyog
District Hospital and Rural Health Unit for one year as a Nurse. With my knowledge and skills I
had gained in my studies and in the practice of my profession, I can assure you that I can be an
asset in achieving the mission and vision of the Bureau of Corrections. I am very much willing to
undergo the process in applying the position of Nurse I as prescribed by this institution.
