Grade 7 Band Permission Form

This form must be returned to Ms. Hutchinson no later than __________________________

I, _________________________________ give my son/daughter permission to take part in the grade seven band
program for the 2015/2016 school year. I understand that there are certain costs involved for renting and maintaining
an instrument, supplies, and a band method book, and that my child will remain in the program for the duration of the
school year.
I understand that in order to ensure a balanced ensemble, all children in band must be flexible on which instrument they
play. While they may not get to play their first choice, they will always get to play one of their three chosen
instruments.
I am aware that I am responsible for obtaining an instrument at a local music retailer (such as Long & McQuade Musical
Instruments, Tapestry Music and Tom Lee Music), and that the instrument I rent will be one that has been approved by
Ms. Hutchinson for my son/daughter to play in the band.
I understand that practicing is expected of all band students, and my child will have to practice on regular basis of at
least 20 minutes per day, five days a week in order to be prepared for concerts held throughout the year.
I understand that participation in the concerts is a mandatory part of the class.
I understand that band is indeed a class, and that my child will receive a mark for band on his/her report card.
