PETITION FOR ADOPTION
OF LIZA SANICO WITH APPLICATION FOR CHANGE OF HER
NAME TO LIZA MILAN SANTOS

COMES,
NOW,
THE
PETITIONERS,
thru
their
counsel, and unto this Honorable Court, most
respectfully allege the following, to wit:
1. Petitioner ROLLY M. SANTOS and CYNTHIA A.
SANTOS, 45 years old and 43 years old respectively,
Filipino, married to each other, and residents of
Purok 5, Brgy. Cuatro, Sto. Nino, South Cotabato;
2. Petitioners are in possession of full civil
capacity and legal rights, are of good moral
character, have not been convicted of any crime
involving moral turpitude, and who are emotionally
and psychologically capable of caring for children.
3. Petitioner ROLLY M. SANTOS is a regular
employee at Filfruit Philippines, Inc with an
annual gross income of Php 560,000.84 for calendar
year 2015, as shown by his Bureau of Internal
Revenue Certificate of Compensation Payment, a
machine copy of which is attached hereto as Annex
A. On the other hand, petitioner CYNTHIA A.
SANTOS is employed as a regular public school

teacher by the Department of Education with an

annual gross income of Php 344,567.00 for calendar
year 2015 as shown by her Bureau of Internal
Revenue Certificate of Compensation Payment, a
machine copy of which is likewise attached hereto
as Annex B. Petitioners therefore are in a
position to support and care for their children in
keeping with the means of the family.
4. Petitioners
have
undergone
pre-adoption
services as required by Section 4 of Republic Act
No. 8552, as shown by Certificates of Attendance in
the Adoption and Foster Care Forum hosted by the
Department of Social Welfare and Development.
Machine copies of the Certificates of Attendance
are attached hereto as Annexes C and D.
5. Petitioners desire to adopt the minor child
named LIZA SANICO, who is female, 3 years old,
Filipino, and likewise residing at petitioners
aforementioned address.
6. Minor LIZA SANICO, was born at Block 60, Lot
44, West Rembo, Makati City, on October 5, 2012, to
RODY SANICO and MADELYN SANICO as their fourth
child. Her fact of birth was duly registered with
the Register of Births of the Office of the Civil
Registrar of Makati City, for which a Certificate
of Live Birth was issued, machine copy of which is
hereto attached as Annex E.
7. Petitioners took custody of minor LIZA
SANICO when the latter was only 3 months old and
have since taken care of her as her parents, RODY
SANICO and MADELYN SANICO, being daily wage
earners, can hardly make ends meet for their first
three children.
8. RODY SANICO and MADELYN SANICO have given
their consent to have minor LIZA SANICO adopted by
petitioners in order that she may be well provided
and well taken care of by the latter. Their consent
to the adoption is shown by their Affidavit of
Consent, a machine copy of which is attached hereto
as Annex F.

9. Minor LIZA SANICO is not disqualified by law

or by the Rules of Court to be adopted.
10. Petitioners desire the change of name of
the subject minor from LIZA SANICO to LIZA MILAN
SANTOS in order to perpetuate their family surname
and in order that the subject minor will be known
to all and sundry and for all intents and purposes
as their own child.
11. Minor LIZA SANICO does not own any real,
personal, tangible and intangible property.
12. It is for the best interest and security of
minor LIZA SANICO if she will be considered as a
true and legal child of herein petitioners as the
latter
can
provide
her
with
love,
care,
understanding and security for the full and
harmonious development of her personality.

PRAYER
WHEREFORE,
premises
considered,
it
is
respectfully prayed of this Honorable Court to
grant this instant petition for the adoption by
ROLLY M. SANTOS and CYNTHIA A. SANTOS of minor LIZA
SANICO and for the latters change of name from
LIZA SANICO to LIZA MILAN SANTOS.
Other relief, just and equitable, are likewise
prayed for.
