Description

Sub-Category

Scaffolders were erecting scaffold platform for cable routing at the main deck without wearing full body
harness and not even providing temporary access ladder to go up and down. The height was about 2.5
meters.

Unsafe Act

Lifting gears (chain block, web slings-SEAFOX ) were found at the main deck without current color code

Unsafe Condition

Pad eyes welded on the ceiling at the bow side winch area was observed without a safe working load
markings and certification.
Fire Extinguisher trolley (50 kg DCP) stationed beside the handrails at cantilever deck was blocked by
Hydro test equipment's.
Electrical cord attached on some of the portable oven at the cantilever deck was found bare and terminals
were exposed.

Scaffolders were observed lifting and transferring of scaffold clamps by using plastic container with a rope,
from lower deck to upper deck of the platform area instead of standard material lifting bag with SWL rated
by manufacturer.

During erection of scaffold at ZWSC riser platform area, it was observed that scaffolders were tightening
the clamps on the tubing of the scaffold at elevation without securing and tied off hand tools.

E&I Crew was observed climbing up on an incomplete scaffold at the riser platform area to check the
measurement of the cable tray support, he was ignoring the "Red Tag" that was placed on the ladder
indicating " do not use scaffold".

Scaffolders were carrying out scaffolding erection overboard underneath the bridge nearby riser platform,
it was observed that operative was throwing scaffold clamp in order to pass to his fellow scaffolder who
was on the adjacent side of scaffold to secure the tubing.

An air hose connected with air manifold for the air supply in the hot work habitat at RP Platform (11 mtr.
Elevation) noted with pin hole . Where the pressurized air was leaking out.
Barge emergency shower at port side deck being observed water leaking / dripping from its fitting and
watering on the floor especially at the access way to gangway areas, posing slippery.

Observed one person going to CSP Boat landing area without wearing work vest.

Scaffolders were erecting scaffold area, it was observed that they were using a rope which was placed at
both sides section of the platform ledger and was secured on the structure in an angular position where
they inserted a piece of pipe in the middle of the rope and applied torsion in order to tighten the rope for
rising the platform to install the scaffold bracing support.

Waste oil tank ( ID NO. OT-94-01) mobilized from NPCC base to barge AL-BURJ for the waste oil collection. "
Observed that its discharge line /tap end was plugged up by inserted a wooden spike /piece instead of

Observed that some loose materials (wooden pallets etc.) were off loaded from the barge Al-Burj to supply

During my routine visit at platform, it was observed that Gas cylinders( One oxygen & two acetylene )
belongs to contractors were observed free standing , not secure with chains or other securing device at
GG2 Platform nearby stair case area, could have a fall hazard.

Fabricator was grinding a welding support inside the habitat at CSP area without wearing the face shield to

Hand tools - Right angle and other tools were observed on the scaffold platform elevation at RP, could

Chain block was observed attached and hang with a 4" Pipe line close by ceiling of the barge bow side for

Scaffolders were shifting scaffold clamps using lifting bag from 1st level to 2nd level at CSP area , the
clamps then placed on the platform gratings. While scaffolder was picking up the clamps, one clamp went
through the hole on the grating and fell down to the 1st level of platform.
Safety latch on 1.5 ton "come along" was observed defective at the 2nd level of CSP area.
A pipe fitter was observed outside the handrail at B-4 Bridge top side area where he was pulling a 1/2"
pneumatic hose to connect to the air blower in the habitat in preparation for GRE pipe support without
wearing body harness and work vest.

A Person was observed carrying materials (two air blowers) in two hands and descending through the stair
at Riser platform (RP).

Unsafe Act

A Fabricator was observed replacing the disc on the grinder without unplugging the power source and not
using proper tools. He was also performing grinding operations using the cutting disc instead of a grinding
disc.

Three scaffolding crew were observed carrying scaffolding clamps on both of their hands while stepping down on the stairway at WIP in
direction of Riser Platform.

E&I Crew was observed climbing in between the twin existing vertical cable tray and stood on the horizontal angular steel support of the
cable tray to cut the plastic tie straps in order to remove the existing cable tray cover at the 2nd level in CSP area without using access
ladder.

Observed that an E & I operative was not wearing ear plugs and respiratory protection while cutting a support with electrical cutter
machine in fabrication shop.

A Fabricator was observed performing grinding operation in the habitat at CSP area without wearing face shield, hand gloves and the
grinder tool that he was using without handle.

NPCC E&I Engineer observed repeatedly coming to mess hall with safety shoes without covers. Already advised several times by crew
and me (HSE Al Burj/Marine) but he still continue the same

During daily routine walkthrough around the barge. Observed at Muster Station No 4; some air hoses/pig tails of H2S Cascade System
were not attached to manifold its hung unintentional and also seen few units the air hoses not available there. The condition are dirty Unsafe Condition
because its let opened without proper cover
NPCC personnel were towing and pushing the trolley with loaded scaffold materials along the narrow pathway between PGB and CSP
area, the person passing by who already stepped aside as he was aware of the approaching trolley almost struck his knee against the
trolley when the trolley swerved to his position.

Observed an E&I worker was over reached and leaned to one side on stepladder as he was intended to tighten a cable tag.
Furthermore the ladder was not secured as standing freely nor griped by other operatives.

The aluminum ladder was brought from barge (AL -Bur) and being used by project personnel despite being informed its against ADMA
Regulations ADMA HSE Reg-102 Chapter 18 Section 5.3.3.C.

Operatives were performing grinding and welding operations on the GRE pipe support at the upper level of scaffold in WIP area. During
inspection inside the habitat, it was found out that the angle grinder electrical cord terminals were exposed and the handle on the unit
was removed.

Poor housekeeping was observed on the scaffold platform underneath the 1st level of CSP. Rubbish (rust of corroded metal) was left
unattended from the existing cable trays which was removed and replaced by E&I
