By virtue of the authority vested in me by Rule 112 of the Revised Rules on Criminal
Procedure, you are hereby notified that a case for MURDER has been filed against you by
MARK S. BUNTAG of Block 5, Lot 2, Phase 4, Camella Homes, Naga City.
Attached is a copy of the complaint and affidavit of witnesses. The documents and other
evidence in support of the complaint are in our Office available for your inspection at anytime
within office hours.
You are granted ten (10) days from receipt hereof within which to submit counteraffidavits and other supporting documents. Said counter-affidavits must be sworn to before any
fiscal, state prosecutor, government official authorized to administer oath or in their absence or
unavailability, a notary public, who must certify that he has personally examined the affiants and
that he is satisfied that they voluntarily executed and understood their affidavits. Copies thereof,
must be furnished herein complainant.
