Previous audit effectiveness:

1. Cleaning personnel is reporting to Mr. Franklin after regular cleaning.


Cleaning kit available and found adequate. Thus CA was effective.
2. Verified painting personnels safety awareness level, found
satisfactory. Mr. Devaraj also monitoring personnel PPE usage
throughout.
3. Machine guard is available with bleaching machine. Thus CA is
effective and found adequate.
4. Ms. Jeya Sudha Packer, received rupees 230 as per day salary for
the month of November. This evident minimum wages compliance.
5. Age proof certificate verified for Ms. Usha Packer, available and
Ms. Muthumari also aware on this requirement.

(Clause 3.1)
Mr. Augustin stacked mats in sample mat area and fire extinguisher
seems unreachable.
Correction:
Stacked mats moved from the pathway instantly and made fire
extinguisher, easy to get to.
Root cause:
Lack of awareness on fire safety and importance of easy accessibility
of fire fighting equipments.
CA:
SAR elucidated on above requirements and its importance to all
personnel. Also insisted in-charges to ensure the same throughout the
processes and accepted.
Effectiveness: 16-06-2015
All firefighting equipments in shop floor kept in right position and easily
accessible. Overall knowledge on importance of fire safety enhanced.

(Clause 3.1)
Slippery floor due to water leakage in gents toilet.
Correction:
Sweeper Mr. Karupaiya drained the floor and detained further slippage.
Mr. Ranjith tightens the flawed valve.
Root cause:
Cleaning person not communicated the dispute on time to concern.
CA:
Cleaning person advised to report Mr. Ranjith if any disputes noticed
during their routines. If not attended/accomplished on time, he/she
shall initiate and pass out information to Mr. Franklin.

Effectiveness: 16-06-2015
All leaky tapes are arrested and awareness on environmental &
personnel safety improved. Consequently this CA found satisfactory.

(Clause 3.8)
Gents dining room made up of grass which may lead fire hazard.
Correction:
Fire extinguisher provided in apposite area. Personnel advised about
new fire extinguisher location.
Root cause:
Lack of awareness about this risk.
CA:
Insisted MP to construct new dining room (brick build). MP endorsed
and insisted Mr. Ponpandi to accomplish the same before 25 th July
2015.
