    DECLARATION ACCEPTING OFFER FOR PERSONAL INJURY

    I Ms Justyna Sochacka of Flat 16, Bamburgh House Park Lane London N17 0HS
    Will say as follows:1. I was involved in an accident on 01-Feb-2016
    2. An offer in the sum of 4000.00 in full and final settlement of my personal injury claim
    has been put forward by the defendants insurance company.
    3. In accordance with the agreement I entered into at the outset with UK Prestige
    Claims Management Ltd, I accept that the above figure is subject to a 20%
    deduction, which is 800.00.
    4. I wish to accept this offer.
    5. Therefore, I agree that I will be receiving a net sum of 3200.00 in full and final
    settlement of my personal injury claim arising out of the above mentioned accident.
