This inspection was completed on April 6, 2016. The following comments are noted with regard to: scope of inspection,
deficiencies and discussions at the workplace.
SCOPE OF INSPECTION
The owner aided in an inspection with Officer Anna Billingsley (OSO) at 11:00 am for the composting facility. The composting
facility is in the final stages of construction and has started compost trails.

The brown water holding ponds do not have guard rails to prevent workers from accessing the lined ponds.
This is in contravention of the Occupational Health and Safety Regulation Section 4.59(1).
A pit or other opening in a floor, walkway, roof or other area accessible to workers, which is a danger to workers, must be
securely covered with a cover of adequate size and strength or guarded by fixed or movable guardrails, which must be identified
as such and kept in place except when necessarily removed to work in the opening or pit.

The employer must ensure that the lined ponds are adequately guarded to prevent workers from access.

The employer has not developed or implemented an Exposure Control plan for hazardous contaminant in their composting
facility. The workers did not have H2S monitors, safe work procedures, and emergency procedures to ensure the safety of
workers.
This is in contravention of the Occupational Health and Safety Regulation Section 5.54(1)(a).
An exposure control plan must be implemented when exposure monitoring under Subsection 5.53(3) indicates that a worker is or
may be exposed to an air contaminant in excess of 50% of its exposure limit.
Implement an exposure control plan which incorporates the following elements:
(a) a statement of purpose and responsibilities;
(b) risk identification, assessment and control;
(c) education and training;
(d) written work procedures, as required;
(e) hygiene facilities and decontamination procedures, as required;
(f) health monitoring, as required;
(g) documentation, as required.
Measures to Ensure Compliance:
The employer must develop and implement an Exposure Control plan for the compost facility.

This is in contravention of the Occupational Health and Safety Regulation Section 9.2.
The employer must:
(a) ensure that each confined space in the workplace is identified, and
(b) determine whether any such space will require entry by a worker, either in scheduled work activities or as a result of
foreseeable system failures or other emergencies.

At the time of inspection a worker removed a manhole cover to access a confined space, however, did not enter the space. The
worker was not aware of the hazards in the confined space and did not know they were not authorized to enter the confined
space.

This is in contravention of the Occupational Health and Safety Regulation Section 9.3.
If a confined space exists at a workplace but no worker entry is required, the employer must ensure that each point of access to
the confined space is secured against entry or identified by a sign or other effective means which indicates the nature of the
hazard and the prohibition of entry, and that workers are instructed not to enter.
Measures to Ensure Compliance:
The employer must ensure that any confined spaces are secured from workers accessing entry into confined spaces.

This inspection was conducted on April 6, 2016. The following comments are noted with regard to: scope of inspection,
deficiencies and discussions at the workplace.
SCOPE OF INSPECTION
On April 6, 2016 2:45 pm, the owner of this farm, Occupational Safety Officer Anna Billingsley, Senior Regional Officer Carol-Anne
Boufford, and Occupational Hygiene Officer Jennifer Shum attended this site for an inspection. The employer is starting up a
composting facility and is currently in its trial runs. This inspection focused on the phase 1 building and the surrounding work
areas associated with the brown water process (eg. brown water storage, above ground storage tanks and sumps).
PHASE 1 BUILDING
-The phase 1 building has four bunkers. Across from the bunker doors, on the other side of the building, is a hopper for brown
water addition. The hopper is then connected via a conveyor system to move brown water- soaked compost back into the
bunkers. At the time of inspection the following represented the state and door positioning of each bunker:
Bunker 1: empty, doors open
Bunker 2: compost process initiated 12 days prior, doors closed (employer is actively trying to move product from this bunker to
bunker 4, but is experiencing technical difficulties)
Bunker 3: compost process initiated 3 days prior, doors closed (employer expressed that they are planning to turn this batch the
next day [April 7] by moving it into the hopper where brown water is to be added).
Bunker 4: compost process nearing completion (2 more days to go from the time of inspection), doors open.
REQUIREMENTS FOR TURNING
-The employer stated that the turning is typically conducted by a worker in a front end loader (enclosed cab).
-The employer has not conducted any risk assessment into the potential for over exposure to hydrogen sulphide (H2S) or other
contaminants in this area. The employer was not able to provide us with any ventilation specifications to determine efficiency for
the bunker ventilation. The employer stated that the worker(s) in the area are not equipped with any personal monitors. As such,
the employer has not ensured that worker exposures to H2S are adequately controlled. See order 1.
-Discussed that prior to turning begins, the requirement to ensure that a risk assessment is conducted and proper procedures
developed for the compost turning process. Discussed that workers are to be equipped with properly functioning four gas
monitors and must be educated and trained in using the monitors.
HIGH LEVELS OF HYDROGEN SULPHIDE DETECTED DURING INSPECTION
-As we exited the phase 1 building, personal monitors worn by WorkSafeBC officers indicated levels of H2S. There were two
exposure events:
1) Officers were standing approximately 2 metres from the bunker doors, between bunker 2 and 3. Levels were detected that
exceeded the allowable limit of H2S.
2) Officers were standing near the entrance and walking outside. Levels were detected again that exceed the allowable limit of
H2S
-Further discussions revealed that a contractor had opened a manhole (accessing they air lock pits). The air locks have been

accumulating residual stagnant brown water. This potentially caused the high alarm events on WorkSafeBC officers monitors.
-As such, the employer has not ensured that exposures to H2S are adequately controlled. Discussed further investigation into the
air lock pits required prior to re-entering the Phase 1 building. See order 1.

The employer has not implemented adequate controls for the accumulation of hydrogen sulphide (H2S) and to prevent worker
exposures to H2S. At the time of inspection, levels detected on personal monitors were observed over the allowable limit. The
employer currently does not have H2S monitors or safe work procedures for workers who are involved with work activities which
pose a risk of over exposures to H2S.
The following are examples of this violation:
- the employer plans to conduct compost turning without first ensuring that H2S is adequately controlled. No H2S monitors were
used, bunker ventilation specifications were unavailable, safe work procedures were unavailable.
-the employers equipment and systems has allowed the accumulation of H2S in stagnant brown water in air lock pits. The
employers contractors while trouble shooting had opened the air lock pits, releasing H2S in the Phase 1 building where workers
were working.
As such the employer has not controlled H2S below harmful levels and below the exposure limit of H2S.

This is in contravention of the Occupational Health and Safety Regulation Section 5.55(1)
If there is a risk to a worker from exposure to a hazardous substance by any route of exposure, the employer must eliminate the
exposure, or otherwise control it below harmful levels and below the applicable exposure limit established under section 5.48 by
(a) substitution,
(b) engineering control, or
(c) administrative control,
(d) personal protective equipment.
Measures to Ensure Compliance:
ALL PERSONS ARE TO STOP WORK (with the exception of persons who are qualified to control this hazard and who are
protected from hazardous exposures) at the Phase 1 building and all areas associated with the brown water process (eg brown
water storage tanks) at this worksite, AND NO ENTRY FOR ANY REASON SHALL BE PERMITTED until this stop work order is
cancelled by this officer.
The employer must develop and implement safe work procedures (based on a risk assessment) associated with an Exposure
Control plan, before any work can resume in the Phase 1 building. The risk assessment and safe work procedures must be
developed by a qualified person and must encompass all potential H2S hazards and exposures within the Phase 1 building,
including but not limited to: the H2S accumulating in and released from the air lock tanks, as well as the H2S exposures from
turning compost.

not limited to, the employer is required to provide the following additional information:
- Complete engineering drawings from all engineers involved with the facility design and construction, including as built
drawings and schematics.
- Who is providing the training on the multi - gas monitors and procedures for the workers.
- Who will be educating the workers on the risk assessment.
- What are the site specific procedures the employers hazardous materials contractor will be following when they enter the
restricted area and what specific work tasks will they be carrying out.
- What type of respirators will workers be wearing.
- What is the percentage of fresh water / brown water being used when the compost goes into the hopper and during other
process steps?
- How many workers will be involved in the work procedures and which workers will be restricted from entering the area?
- Emergency and first aid procedures.
- Use of signage and/or barriers to prevent other workers from entering the area while this procedure is being followed.
- The procedures are required to detail the specific tasks the workers will be required to carry out.

Initiating order:
Apr 06, 2016 - The employer has not implemented adequate controls for the accumulation of hydrogen sulphide (H2S) and to
prevent worker exposures to H2S. At the time of inspection, levels detected on personal monitors were observed over the
allowable limit. The employer currently does not have H2S monitors or safe work procedures for workers who are involved with
work activities which pose a risk of over exposures to H2S.
The following are examples of this violation:
- the employer plans to conduct compost turning without first ensuring that H2S is adequately controlled. No H2S monitors were
used, bunker ventilation specifications were unavailable, safe work procedures were unavailable.
-the employers equipment and systems has allowed the accumulation of H2S in stagnant brown water in air lock pits. The
employers contractors while trouble shooting had opened the air lock pits, releasing H2S in the Phase 1 building where workers
were working.
As such the employer has not controlled H2S below harmful levels and below the exposure limit of H2S.

This is in contravention of the Occupational Health and Safety Regulation Section 5.55(1)
If there is a risk to a worker from exposure to a hazardous substance by any route of exposure, the employer must eliminate the
exposure, or otherwise control it below harmful levels and below the applicable exposure limit established under section 5.48 by
(a) substitution,
(b) engineering control, or
(c) administrative control,
(d) personal protective equipment.
Measures to Ensure Compliance:
ALL PERSONS ARE TO STOP WORK (with the exception of persons who are qualified to control this hazard and who are
protected from hazardous exposures) at the Phase 1 building and all areas associated with the brown water process (eg brown
water storage tanks) at this worksite, AND NO ENTRY FOR ANY REASON SHALL BE PERMITTED until this stop work order is
cancelled by this officer.
The employer must develop and implement safe work procedures (based on a risk assessment) associated with an Exposure
Control plan, before any work can resume in the Phase 1 building. The risk assessment and safe work procedures must be
developed by a qualified person and must encompass all potential H2S hazards and exposures within the Phase 1 building,
including but not limited to: the H2S accumulating in and released from the air lock tanks, as well as the H2S exposures from
turning compost.

Apr 06, 2016 - Based upon the violation cited in this Inspection Report, the Board has reasonable grounds to believe there is a
high risk of serious injury, serious illness or death to a worker at this workplace. Workers are at risk of exposure to H2S and other
air contaminants due to inadequate controls in place.
The Board, in accordance with subsection (1.1), may order that work at a workplace or any part of a workplace stop until the
order to stop work is cancelled by the Board.
This stop work order is in effect as of April 6, 2016 4:30 pm, and in accordance with section 191 (4) of the Workers
Compensation Act, expires 72 hours from this time, unless this order is confirmed in writing by WorkSafeBC.

This inspection report relates to Confirmation Stop Work Order issued on inspection report 201616321043B on April 8, 2016.
****THIS STOP WORK ORDER CONFIRMATION IS STILL IN EFFECT*****
Written safe work procedures dated April 9, 2016 and in the form of a Job Safety Analysis (JSA) Hazard Identification was
submitted to this Officer for review and acceptance on April 9, 2016. This JSA was prepared by the employer's hazardous
materials contractor. In addition, the employer submitted a risk assessment report dated April 7, 2016 that was prepared by the
employer's health & safety consultant.
The development and implementation of these procedures and risk assessment are permitting the employer to carry out work that
is necessary or required to remove the danger or the hazard and only if workers are protected from the danger or the hazard or is
qualified and properly instructed in how to remedy the unsafe condition with minimum risk to the worker's own health or safety as
required by Workers' Compensation Act 191 (2).
The employer understands that these procedures are only for carrying out the task of turning over mushroom compost material in
bunker two and moving mushroom compost material to the hopper to mix with fresh water and other additives (i.e. chicken manure
and gypsum) and moving the mushroom compost material to bunker four. No other work activities will be carried out as identified
by the Stop Work Order Confirmation.
During this procedure if the employer identifies significant levels of Hydrogen Sulphide (H2S) in air (which is not to be expected
based on the preliminary risk assessment) they are to notify WorkSafeBC immediately.
The employer has been requested to provide WorkSafeBC with the completed "as built" drawings of the Phase 1 and Phase 2
process buildings as well as all the engineering design schematics. This information is important for the employer to complete
their risk assessment and plan their next steps to address the brown water which has become anaerobic and which has collected
in the air-lock pits. This anaerobic brown water continues to present a high risk of H2S exposure to workers. In addition, this
information will be important for the employer's health & safety consultant to complete the exposure control plan for the air
contaminants (e.g. ammonia, H2S, CO2, etc.).
Any new procedures or revised procedures will need to be submitted to WorkSafeBC for review as it relates to the Stop Work
Order Confirmation.

Written safe work procedures dated April 11, 2016 and in the form of a Job Safety Analysis (JSA) Hazard Identification was
submitted to this Officer for review and acceptance on April 11, 2016. This JSA was prepared by the employer's health & safety
consultant. In addition, the employer's hazardous materials contractor submitted to WorkSafeBC their field work flow document on
the work activities carried out on April 10, 2016. This report identifies that no readings of H2S were detected during their work
activity.
The employer has revised their procedures based on air monitoring results that were taken on April 10, 2016 by their hazardous
materials contractor. In addition, the employer's health & safety consultant is now on site assisting with the revised procedures
and work tasks.
In addition, the employer has hired a Construction Safety Officer to assist them with site coordination and to provide first aid
services.
The development and implementation of these procedures and risk assessment are permitting the employer to carry out work that
is necessary or required to remove the danger or the hazard and only if workers are protected from the danger or the hazard or is
qualified and properly instructed in how to remedy the unsafe condition with minimum risk to the worker's own health or safety as
required by Workers' Compensation Act 191 (2).
