Student Behavior Contract

TO: PARENT/GUARDIAN AND STUDENT
_________________________________will begin this contract on this date _______________ to help in assuring his/her
success. This contract will be used to assist in determining their future at Okmulgee Elementary School.

GOALS FOR STUDENT:

1.
The student will follow all school rules and behavior expectations at Okmulgee Elementary School.
2.
The student will be a successful student.
3.
The student will complete a successful elementary school experience.
REWARDS IF GOALS ARE MET:
1.
The student will be allowed to stay in school for a full day of experiences and instruction.
2.
The student will have social time with peers of his/her own age.
3.
The student will have an education equal to his/her peers.
4.
The student will have all the privileges that students enjoy.
CONSEQUENCES IF GOALS ARE NOT MET:
1. The student will be sent home each and every time discipline referral is sent and accepted by the principal.
2. The next referral will result in 3 days home, the next 5 days home and the next will result in the student
only attending core classes.
3. The next referral will result in full time home instruction. The school will provide a Friday Folder that will
be picked up by the parent on Friday and work returned the next Friday, when the new work is picked up.
The student will no longer be on campus for their instructional day.
Special Note: The rewards and consequences mentioned above are null and void if student commits an act that would require a
suspension and/or expulsion. This will be determined by school policy and school administration.

By signing this contract all parties agree to the stipulations in the document and will follow accordingly.
