AFFIDAVIT OF DENIAL
I, DEXTER CRUZ REYES, of legal age, Filipino, and with residence at No.
Block 11, Lot 38, Yakal Street, Phase 4, Multinational Village, Paranaque City, Metro
Manila, after being sworn to in accordance with law, depose and states:
1)

I have applied for a National Bureau of Investigation Clearance and

appeared that a certain Dexter Cruz Reyes is charged for Violation of Sec. 1119 RO
(Traffic Obstruction) CC# 06-241349 RTC Manila 12/19/2005.
2)

That I deny the said alleged charges against me and that I am not the

person charged for Violation of Sec. 1119 RO (Traffic Obstruction) CC# 06-241349 RTC
Manila 12/19/2005. as stated in Par. 1 hereof.
3)

That the Information in the said above stated case has no connection with

my identity or personality.
3)

I have executed this Affidavit of Denial in connection with my application

for Clearance from the National Bureau of Investigation and to attest under oath
the truth of the foregoing and for all legal purposes and intents it may be worth.
