address at Tukananes, Cotabato City, after being duly sworn to in accordance with law,
hereby depose and say:
1. I am a member of Armed Forces of the Philippines-Philippine Army since
December 7, 1998 with a rank of Corporal;
2. I am assigned at 66IB New Bataan, Compostella Valley;
3. I am ready and willing to assume and fulfill my duty and responsibility as a
member of the Philippine Army;
4. I am likewise solemnly promising and undertaking that I will perform my duty
as a soldier upon the approval of my Application for Extension for service.
