DAMAGE INSPECTION REPORT

APPLICANT: WALLEM VIETNAM
VESSELS NAME: M/V DIRECTION S/W
PLACE OF INSPECTION: DA NANG PORT, VIETNAM

At the request of BAO VIET INSURANCE; We - surveyor of


VINACONTROL DA NANG present at the M/V DIRECTION S/W at 0800 hrs
on 15/10/2016 for damage inspection. We found that:
At 0912 hrs on 13th, OCT. 2016 the M/V DIRECTION S/W berth at the
JETTY No1 Danang PORT.
At 0700hrs OCT., 2016, the representative of Da Nang PORT noticed
that the flange which is attached to fender No1 on berth Tien Sa 1 was
broken on the right corner (dimension: 50x200 cm). According to Port
operator, the impact between M/V DIRECTION S/W and the jetty in strong
wind condiotion was the cause of this incident (in was level 6-7 wind from
18:00hrs 13/10 to 0300hrs 14/10 at Da Nang Port and the port autrority
requested this vessel to departure but is still berthed).
According to Master, when this vessel berthed at Da Nang port, the
wind scale was only 3-4, gust was 4-5. And the flange of the jetty was
already old.
After checking and taking the pictures of related evidences, we
described the damage as follows( pictures attached) :
The concrete layer of flange was broken and seperated from the jetty
The rubber fender in normal condition.
Two bolts which is used to attach the fender to the flange were lost and
the other one was loosen.
The collision also caused a scratch length 1,85m on the hull of this
vessel.
The reason and extent of the damage wil be determined afterwards.
