Vicinity Map (with a minimum of 2 kilometre - radius from the periphery of the
project site showing the relationship of the proposed project to existing

Site Development Plan (Schematic Plan) showing the proposed lay-out (For
projects in Baguio City and Benguet, plot all existing trees with a diameter of
0.30 meters or more)
Survey plan of the lot/s as described in the Certificates of Titles of lots subject of
the application
Certified True Copy of titles and Tax Declaration (certification should not be more
than three (3) months old upon filing of application)
Permit to Cut Trees (For projects located in Baguio City and Benguet)
Topographic Plan to include existing conditions like boundary lines, streets and
basements / utilities and adjacent to the project ground / spot elevation and
other conditions on the land

When certified copy of the TCT was issued more than one month prior to the
application for Certificate of Registration (CR) affidavit of the owner that the
property is free from liens and encumbrances. When the applicant is not the
owner deed from the registered owner showing clearly the authority of the
applicant to the property, e.g its development and sale of individual lots or
units; to sign and receive REQUIRED DOCUMENTS; and other relevant authority.
In case the subdivision / condominium project or portion thereof is mortgaged.
In case the project or portion thereof is mortgaged, it is sufficient if the
instrument of mortgage contains a stipulation that the mortgagee shall release
the mortgage or any subdivision lot or unit as soon as the full purchase price for
