AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MIGUEL A. KISWA and JOSEPHINE BALUSDAN, Barangay Chairman
and Barangay Kagawad respectively, of Santo Tomas Proper Barangay,
Baguio City after having been duly sworn to in accordance with law, do
hereby state the following to wit:
1. That we are presently Barangay Officials of Santo Tomas Proper, Baguio
City.
2. That we personally know MR. JIMMY A. MANIAGO who is a bonafide
resident of this Barangay with residence and postal address at
Balacbac Central, Santo Tomas Proper, Baguio City since 1980s.
3. That we knew and certify that said MR. JIMMY A. MANIAGO and MR.
SANTIAGO A. MANIAGO is the same person.
4. That we are executing this affidavit to attest to the foregoing facts and
statements.
