AFFIDAVIT OF CONSENT
I, MILAGROS PIAZA-BASISTA of legal age, married, a Filipino citizen,
and a resident of Garcia Extension Access Road, Brgy. Narra, Catarman, Northern
Samar, after having been duly sworn to in accordance with law do hereby depose
and say:
1 That I am the lawful and registered owner of a Savings Account with
Account Number 346-3-34619716-2 in Metrobank, Catarman
Branch;
2 That I am indebted to a certain some of money to one Robert Cho Bolos,
also of legal age, married, and resident of Brgy. Molave, Catarman, Northern
Samar;
3 That in view of my said indebtedness to Robert Cho Bolos, I give my full
consent without any conditions, the authority to one Robert Cho Bolos to
encash and/or claim the money under the two (2) Metrobank Checks issued
by Turbanada Inc., pay to the order under my name dated August 31, 2017
with Check No. 3461770349 and February 28, 2018 with Check No.
3461770355 all contain Two Hundred Thousand Pesos each as payment of
my loan to him;
4 That I am executing this affidavit to attest to the truth of the foregoing
statement of facts for whatever legal purposes this may serve.
IN WITNESS WHEREOF, the undersigned affixed her signature above her
name this 9th day of June 2016 here at Catarman, Northern Samar, Philippines.
