GUIDELINES:
1. All NSTP students are required to vote during the SSC KASAMA elections as scheduled by the SSC
COMELEC. Also, each NSTP student is required to scout and involve at least three more SLU students
who are not currently enrolled in the NSTP to vote. Each enrolled NSTP student must have 3 successfully
and properly accomplished forms to submit and this will be equivalent to 20/20 quiz points. Students who
will not complete the said 3 forms will only be given a score of 2/20. As part of this requirement, NSTP
students should therefore accomplish this voters certification.
2. Requiring students to vote during the SSC KASAMA election is among the activities of SLU NSTP in
promoting student leadership, participation and consciousness, and in actively involving students as youth
in nation building beginning in the university community, exercising our responsibility to vote.
3. Download this form from the SLU NSTP FIRST SEMESTER FACEBOOK PAGE and have it reproduced
for your use as instructed by your instructor.
4. Secure a copy and bring it along to the voting precinct when you actually vote. Distribute copies also to
your scouted voters. This form must be accomplished or processed personally by the concerned NSTP
student and the scouted students to vote during the voting itself.
5. Process one form per student only; no duplication of forms for one student is honored.
6. As you vote in the precinct, show this to the election supervisor in the precinct. Fill up first the necessary
blanks before you present this form to the election supervisor.
7. Only forms with original handwritten data and signatures will be honored by the instructor. Photocopied
data and signatures are null and void.
8. After voting, the NSTP student must compile his/her accomplished/processed form together with the forms
of his/her scouted non-NSTP students. First paper in the compilation should be the form of the concerned
NSTP student. The attached forms are that of the scouted students.
9. Additional scouted voters in excess of the required five non-NSTP enrolled students will be considered
merit points. One person is one raw merit with a total not exceeding ten raw merits per NSTP student for
this activity.
10. Submit your forms to your respective NSTP instructor during your NSTP classes (Monday/Tuesday)
immediately after the election. For NSTP 2 students, submit your forms to your respective instructors.
11. Only the assigned election supervisor for a particular precinct the student is voting is the authorized
signatory of this form.
12. Signing of this form by the authorized SSC COMELEC election supervisor must be during the voting time
of the concerned students. NO voting or no form during the actual voting, NO signing by the election
supervisor.
