    While walking from his home to Bhatia hospital on September 12th, 2016, Ashok Bhansali lost his wallet near Forjett Street, which contained his original driving license numbered 2004/C/33730 and election card numbered JRW1085349. He wrote to the police officer in charge of the Tardeo Police Station to register an FIR regarding the loss in order to prevent misuse of the licenses, and enclosed copies of his lost driving license and Aadhar card as identification proof.

    Respected Sir,
    While travelling on Monday 12th September,2016 from my house to Bhatia hospital by walking I
    have lost my Wallet which had my Original Driving License bearing No.2004/C/33730 and
    Election Card bearing No. JRW1085349, somewhere near Forjett street area. Sir, to avoid any
    misuse of my above mentioned license I seek your help and request you to kindly register my F.I.R
    in the subject matter.
    For your ready reference I enclose herewith
    (a) copy of lost Driving License
    (b) copy of Aadhar Card as I.D proof.
    Hope you will do the needful favorably at the earliest.
