    Consent Application

    How to Use:
    Note: In order to make the consent application work, please make sure that the
    CONCENT CD is kept inside the CD Drive of your computer.
    Search Consent: User can type Consent Title on search box and application will start sorting
    Consent list.

    Manage and print View: User can manage view of consent and Print Consent using top bar.

    Pre fill Hospital related Information: By using Hospital Info tab User can prefill Hospital
    related information into selected Consent form

    If Consent is getting printed on Letter head then user has to select First Option i.e. Print on
    Hospital / Facility / Doctors Letter head. And give proper margin to adjust consent print on
    letter head.
    If User is printing on plain A4 or Letter sized Paper and want to define Hospital / Facility name,
    address, Reg no. then he/she can use second option i.e. Print on simple paper.
    Patient Info: To prefill patient related information on Consent user can use Patient info tab
