5. 2 copies of Affidavit of Non-Employment (FOR UNEMPLOYED DOCUMENT
OWNER) INDICATE: Purpose For Change of Name and No pending
6. 2 latest original copies of NBI clearance

Purpose: for change of name

7. 2 latest original copies of PNP clearance

Purpose: for change of name

8. 2 copies of valid ID of petitioner and document owner and 1 copy of latest

community Tax certificate from the place of work or residence
9. Any of the following documents, only when available:
a. Diploma (Elem., HS or College);
b. Medical Record
c. Insurance;
d. Certificate of Land Title;
e. Passbook
