o Frequent, loose stools.
6. Diagnosis IBS dan patogenesis
IBS pernah dianggap sebagai "diagnosis eksklusi" dan karena itu,

Various functions of gastrointestinal system in nutrient assimilation are:
a. Digestion : Breakdown of nutrients ingested to simplest form that can be

b. Secretion : secretion of digestive juices to gastrointestinal lumen by
glands along the food pathway in gastrointestinal tract, particularly in oral
cavity, gaster, and small intestines.
c. Absorption :absorption of water,various electrolytes, and digestive
products in small and large intestines. Digestive and absorption process is
explained in anwer no. 7
d. Motility : Ensure movement of food through the alimentary ract. There
are 2 general pattern of motility; peristalsis and segmentation & mixing.
Peristalsis is a reflex response that is initiated when the gut wall is
stretched by the contents of the lumen, and it occurs in all parts of the
gastrointestinal tract from the esophagus to the rectum. The stretch
initiates a circular contraction behind the stimulus and an area of
relaxation in front of it. The wave of contraction then moves in an oral-tocaudal direction, propelling the contents of the lumen forward at rates
thatvary from 2 to 25 cm/s. Peristaltic activity can be increased or
decreased by the autonomic input to the gut, but its occurrence is
independent of the extrinsic innervation.

Almost all of the fats absorbed from the intestinal tract are not carried in the
portal blood but instead are absorbed into the intestinal lymphatics and then
conducted to the systemic circulating blood by way of the thoracic duct,
bypassing the liver.

Digestion starts in the mouth, equipped with salivary gland that produce
saliva that contains antibacterial compounds and various enzymes to aid the
breakdown of food molecules. Teeth chop and grind food, breaking the food

area over which the digestive enzymes in saliva can act. Food is then
swallowed and passes into the pharynx then propelled through the
esophagus and into the stomach by means of muscular contractions called
peristalsis. The stomach aids digestion in two ways. Its strong muscular walls
churn the food into chyme while glands within the walls secrete gastric juice.
As the semi-digested food (chyme) enters the duodenum from the stomach,
the duodenal lining releases intestinal hormones that stimulate the
gallbladder and pancreas to release special digestive juices (bile and
pancreatic juice) which help to further break down food molecules in the
chyme. It is in the small intestine that most nutrients are digested and
absorbed, although different nutrients are absorbed at different speeds. The
duodenum and jejunum is where the chyme is broken down, while the ileum
is responsible for absorbing nutrients into the bloodstream. The absorbed
nutrients pass through the bloodstream to the liver where they are processed
and either stored or distributed to other parts of the body. After every useful,
digestible ingredient other than water has been wrung out of the chyme, the
remaining "waste" passes into the large intestine.

reason: Digestive enzymes act only on the surfaces of food particles.
Swallowing (deglutition)
Swallowing is a complicated mechanism, principally because the pharynx
sub-serves respiration as well as swallowing. The pharynx is converted for
only a few seconds at a time into a tract for propulsion of food. It is
especially important that respiration not be compromised because of
swallowing. In general, swallowing can be divided into (1) a voluntary stage,
which initiates the swallowing process; (2) a pharyngeal stage, which is
involuntary and constitutes passage of food through the pharynx into the
esophagus; and (3) an esophageal stage, another involuntary phase that
transports food from the pharynx to the stomach.
b. Digestion
The aim of this process is breaking down macronutrient (carbohydrates, lipid
and protein) becomes smaller then can be absorbed in GIT.
Digestion of carbohydrates in mouth and stomach
When food is chewed, it is mixed with saliva, which contains the digestive
enzyme ptyalin (an a-amylase) secreted mainly by the parotid glands. This
enzyme hydrolyzes
starch into the disaccharide maltose and other small polymers of glucose
that contain three to nine glucose molecules. However, starch digestion
sometimes continues in the body and fundus of the stomach for as long as 1
hour before the food becomes mixed with the stomach secretions.

Absorption in small and large intestine
Absorption from the small intestine each day consists of several hundred
grams of carbohydrates, 100 or more grams of fat, 50 to 100 grams of amino
acids, 50 to 100 grams of ions, and 7 to 8 liters of water. The absorptive
capacity of the normal small intestine is far greater than this: as much as
several kilograms of carbohydrates per day, 500 grams of fat per day, 500 to
700 grams of proteins per day, and 20 or more liters of water per
day.Thelarge intestine can absorb still additional water and ions, although
very few nutrients.
About 1500 milliliters of chyme normally pass through the ileocecal valve
into the large intestine each day. Most of the water and electrolytes in this
chyme are absorbed in the colon, usually leaving less than 100 milliliters of
fluid to be excreted in the feces. Also, essentially all the ions are absorbed,
leaving only 1 to 5 milliequivalents each of sodium and chloride ions to be
lost in the feces. Most of the absorption in the large intestine occurs in the
proximal one half of the colon, giving this portion the name absorbing colon,
whereas the distal colon functions principally for feces storage until a
propitious time for feces excretion and is therefore called the storage colon.
