This document provides instructions and accommodations for a formative assessment on graphing weather for students. It asks students to answer why meteorologists use graphs, and to draw examples of bar graphs, line graphs, and picture graphs. For a student with cerebral palsy, the teacher would assist in reading directions, allow the student to verbally answer the first question with speech therapy help, and provide graph templates on an iPad app for the student to draw the graphs with colors and save their work.

****When giving this formative assessment at the
beginning, middle, and end of the lesson, I will assist students in reading the directions and questions if needed. **** Accommodation: The accommodation for this assessment would be for a student with cerebral palsy. Instead of students writing down the answer to the question in number 1, I could ask them to verbally say it, with the help of a speech therapist. To make the graphs, the student could use an iPad to draw the graph. I could give the basic set up each of the graph, which is inserted below. Then, I could save the template for each graph to the iPad, and insert it to the app, Doodle Buddy. On this app, the student could draw the pictures of the graphs on here, and select different colors to use. The student can then save their work and I could assess it. The student could also take breaks in between making each graph.
