TYPE OF INCIDENCE: Fire Incidence
CHIEF COMPLAINT:
Pt. 1: Pain at right foot and lower back
Pt. 2: Swollen cheek
Pt. 3: Dizziness

Pt. 4: Swollen upper lip


Pt. 5: Swollen left knee and left foot

NARRATIVE REPORT:
At around 0041H, report was confirmed from Bureau of Fire Protection regarding a fire incident
in Blk 2 Lot 3 #8 Medina Compound, Brgy. Talon Kwatro. LPDRRMO team responded to the
area and immediately made a courtesy call and incident site survey.
According to initial report from the Bureau of Fire Protection, the fire which extended to 3 rd
alarm started at around 1230H from the house owned by the family stated below which rapidly
spread affecting 20 houses and 50 families in total. Five patients were catered during the
incident. Patient 1 was transported to Las Pias City Medical Center, Patient 2, 4 and 5 was
given cold compress and Patient 3s blood pressure was monitored by the team.
Responding team returned to base at around 0412H.
