A pplicant: Submit this application and the following additional items to the Lansing City Clerk's
Office, 124 W . Michigan A venue, 9th Floor, Lansing, Michigan 48933-1695; Phone (517) 483-
4133. A pplication must be submitted to the City Clerk with supporting documentation at least
fifteen (15) days in advance of the proposed date of the display.
1. Proof of insurance coverage for public liability of $500,000 for injury to one
person and $1,000,000 for injury to two or more persons resulting from the same
accident. The certificate of insurance or bond shall provide a minimum of
$500,000 property damage and the City shall be named as co-insurer on the
certificate.
2. Proof that the applicant is eighteen years of age.
3. Copy of driver's license of individual in charge of firing the display.
4. Non-refundable fee in the amount of $125.00.
5. Affidavit verifying the information contained in this application.

Basis of Eligibility
As to the eligibility of this project under the OPRA legislation, investigation of the
property on March 23, 2010 by the City Assessor concluded the following:
'The subject property was originally designed/used as a retail store or restaurant with
either supporting office space or living space in the upper floors. The inspection of the
property revealed physical deterioration resulting from rotting floor joists, interior water
damage from leaking roofs and inadequacies of design in location of basement
entrances. The subject property lacks compliance with current ADA (Americans with
Disabilities Act) standards with the only restrooms for the restaurant located in the
basement with no handicap access. The property also suffers from curable and
incurable obsolescence requiring modernization. If the subject property were continue to
be utilized as a restaurant, as it was originally designed, all of the plumbing, heating and
cooling mechanical systems along with the electrical systems would need to be updated
or installed, to provide for efficient operation.

School District summer tax collections. Also enclosed is the Estimated 2010 Summer Tax
Levy Summary, which is being provided at this time for informational purposes only.
For the 2010 tax year Ingham Intermediate School District levied 100%
summer taxes in parallel with its constituent local districts who collect 50%
or 100% of their operating tax levy in the summer. We will collect 100%
winter taxes in parallel with our constituent local districts who collect 100%
of their operating tax levy in the winter. Please take care to see that the
