DearGuardianandStudent,
Thank you for your registration with San Yu Adventist School. We will process your registration and
get back to you within 3 working days if further clarification is needed from you. If you do not hear
from us please prepare to come for the Placement test on your selected date.
It is compulsory for Student and Guardian to attend Pre-course counselling. Without attending Precourse counselling, school will consider you give up your application to other applicants.

Pre-course Counselling and interview will only available by selected date and appointment.
Please ensure that you bring along the following documents for verification when you come to take
the Placement test :
1) Birth Certificate

2) Past 2 years School results

3) Passport
4) Confirmation email
Please prepare to arrive at San Yu Adventist School2 hoursbefore the scheduled time if you
have not verified your documents by San Yu Adventist School staff.
You are not allowed to take your Placement test without complete documents for verification or
Late.
*NOTE :
1) all translated copies must be duly notarized copies.
2) If you are in Singapore, you may bring along your original's documents to San
Yu Adventist School for verification on Monday to Thursday from 10am to 330pm
instead of arrive School 2 hours before schedule placement test time. However,
you will need to arrive at least 1 hour before schedule test time.

RefundInformation
Termsandregulations

1.Theapplicantisrequiredtositfor2placementtestpapers:EnglishandMathematics.

2.TheApplicationfeeisnonrefundableiftheapplicantdoesnotshowupforthetest.Full
paymentistobemadebeforeregistrationtoconfirmparticipationfortheplacementtest.An
officialreceiptwillbeissued(email)totheapplicant.Eachapplicantwillberequiredtobring
alongconfirmationmessage(emailandreceipt)forverificationwiththeirpassportonthe
placementtestdate.

3.Pleasenotethattherewillbenorefund/compensationofanyshouldapplicantdoesnot
meettheplacementteststandardsandrequirements.

4.Pleasenotethatnostationerieswillbeprovided.Pleasebringyourownstationerieseg
calculatorandmathematicalinstruments.P1andP2studentswillneedhavecolourpencils
fortheirplacementtest.PS1FandSecondarystudentswillneedtohaveascientificfortheir
placementtest.

5.Mobilephones,dictionaries/electronicdictionariesarenotallowedtobebroughttothe
examinationvenues.

6.Theinvigilatorsreservetherighttoterminate/removeanystudentifhe/sheisfound
cheatingorexhibitirregularitiesduringthetestandwillnotbepermittedtositforanyother
papers.

7.PleasenotethattheSchoolreservesthedecisionmakingrightsintheapprovalofthe
studentsadmissionbasedontheoutcomeoftheplacementtestandinterviews.Noreason
willbegivenfornonacceptance.

8.Pleasenotethattheresultsoftheplacementtestwillbereleased2weeksafterthetest

andnogradewillbegiven,onlyPassorFailwillbeinformedthroughthetelephoneor
email.

9.PleasenotethattheresultofourSchoolplacementtestdoesnotentitletheapplicantto
otherschoolsentryrequirements.

10.Successfulapplicantwillbegiven7schoolworkingdaystoconfirmtheirplacementby
submittingtheApplicationforAdmissionform,failingwhichtheplacemaybegivento
anotherapplicant.TheSchooldoesnotreservetheplacefortheapplicant.

11.GettingaplacementwiththeSchooldoesnotguaranteetheapplicantaStudentPass.
ApplicantmustobtaintheapprovalletterfromICAbeforehe/shecanofficiallysignonasa
studentinourSchool.

12.Formattersnotprovidedforinthisagreement,applicantshavetheresponsibilityto
enquireiftheyareunsure.

Declaration by Applicant/Student
1. I agree to comply with the terms and regulations listed above.
2. I declare that I did not have poor conduct in my previous schools.
3. I declare that I do not have any tattoo on my body.
4. I declare that I do not smoke, drink alcoholic drinks or abuse drugs.
5. I declare that I do not have any criminal record.
6. For P1 to P4 students only. I understand it is compulsory for me to attend the Adventurer Club, a uniform
group, for CCA.
7. I will withdraw myself from the school if I do not fulfill any of the above or is discovered to have contravened
any of the above after being admitted into this school.
8. I understand that the Placement Test will be informed to me as Passed or Failed only and no further
correspondence will be entertained regarding this.
9. I understand that the school does not give any reason to reject my application.
10.I accept that the application fee cannot be refunded in any case.

Declaration by Guardian
As the local guardian of the applicant, I will
1. Beresidentin Singapore,contactableat all times by the student and the school.
2.Notifythe school immediately if there is any problem concerning the student, change of address, contact numbers etc.
3.Informthe parent(s) of the student directly regarding any matter that the school has conveyed to me, and understand that
the school will not be held responsible for the parent(s) ignorance of the matter.
4. Beresponsiblefor any report sent by the school to my address stated on page 7 and will act on or reply to it promptly.
5.Demandfrom the student any test or exam or term report, sign it, return it to the school, and inform the parent(s).
6.Supervisethe student to ensure that the student complies with all school and ICA requirements.
7.Cooperatewith the school concerning the studies, welfare and discipline of the student.

8.Informthe school and the parent(s) of the student if I cease to be the local sponsor/guardian of the student.
9.Informthe school if the student fails to attend school without any valid reason.
10.Informthe school in writing one week in advance if the student is withdrawing from the school.
11.Ensure that the student surrenders his/her Student Pass to the school immediately upon his/her withdrawal from the
school or at any point in time when the school terminates his/her studies for any reasons deemed adequate by the school.
Declaration by parent
1. I understand and accept the policy and conditions governing enrolment and admission of my child into the School and I
acknowledge that withholding relevant information relating to my childs physical, medical or educational needs may affect my
child being offered or maintaining a place within the School. I declare that to the best of my knowledge all of the information
which I have supplied in this application form is full and accurate. I undertake to inform the School immediately of any change in
the particulars to this application or of my wish to withdraw it. There is no refund of fee for such cases.
2. I understand that the Principal may require at any time the withdrawal of my child from the School for any cause judged by
the Principal in his/her absolute discretion to be adequate. Adequate cause may include matters such as my childs
misconduct or my childs inability to participate in and benefit fully from the school curriculum without extensive individual
assistance. It may also include the failure at the stage of applying for a place in the School to declare accurately and fully the
extent of my childs individual educational needs. There is no refund of fee for such cases.
3 I declare that my child is not suffering from any mental impediment or abnormality. I have no objection to my child attending
chapel and Bible Knowledge class.
4. I understand and agree that if I want to withdraw after signing the Student Contract and payment of school fees, the fees will
be refunded in accordance with the Refund Policy stipulated in the Student Contract, and I am not entitled for further refund.
5 For Primary 1 to Primary 4 students. I agree to my childs participation in the compulsory CCA the Adventurer Club.
6. As my families are not living in Singapore, I, hereby, appoint the following person as my childs legal guardian. I agree that
the School will only liaise with my childs legal guardian on the school matters concerning my child.
7. I will inform the school if there is a change in my childs guardian.

I (The guardian / Parent) have read and understood the terms and regulations required. I (The guardian / Parent)
undertake the responsibility to abide to the terms and regulations (for myself, my child or ward) and will not hold the
School responsible for any damages/loss due to misunderstanding and/or miscommunication I (The guardian) have
regarding the terms laid out in this email.
