EXPERIENCE
LAKE RIDGE ASSISTED LIVING
08/2015 02/2016
Working as a CNA, I assisted residents with ADLs and meal
times. I worked with many home health companies and
their staff. I was trained into a med tech position after only
a few months.
SYMBII HOME HEALTH & HOSPICE
12/2014 - 08/2015
I visited a variety of clients in their homes and assisted
with ADLs and IADLs. Clients ranged from post-surgery to
end of life care. I was exposed to working in various
environments and assisting family/caregivers during their
most vulnerable times.
PROVO REHABILATION & NURSING
01/2012 09/2014
I was promoted into a Restorative Nursing Assistant (RNA),
quickly after being hired. I assisted in CNA work; bathing,
toileting, transfers, and grooming. As an RNA, I ran therapy
programs to residents in the long term & memory care
units (ROM, transfers, etc.), ran an excellent feeding
program in the long term care dining room, and
obtained/recorded weekly & monthly weights for about
110 residents.

EDUCATION
AAS/ OCCUPATIONAL THERAPY ASSISTANT
Salt Lake Community College
I have been exposed to a wide variety of clinical and civic
engagement services. I participated in a Wellness Check at
a local senior center in my first semester. I have
experience school-based, sensory integration and ran a
kindergarten class in a handwriting lesson. I have also
worked with community clients with mental disabilities in
several settings. I have participated in the SLCC OTA clinic
for three semesters, which duties include planning,
carrying out treatment plans, and documentation. For
