Softlenscomplicationsareoftenaresultofcontactlens
deposition.Theuseofdisposablelensesespeciallydaily
andtwoweekreplacementmodalitieshashelpedin
decreasingdepositionandtheproblemsassociatedwithit.
Whenusingyearlyorthreemonthreplacementmodalities,
however,patientsmustperformproperlenscaretoprovide
acleanlenssurface.Hydrogellensesdepositsecondaryto
theirhydrophilicsurface,patienttearfilm,environmentand
lenshandling.Herearesomecommontypesofsoftcontact
lensdepositsandhowtomanagethem.

Oneofthemorecommondeposits,proteindeposits
originateprimarilyfromalbumin,globulinandlysozymein
thetears.Aproteindeposithasanopaque,whitefilmy
appearanceandmayhavecrackswhenthefilmisthick.It's
foundprimarilyonthefrontsurfaceofsoftcontactlensesandonbothsurfacesofGPlenses.
Depositionbeginsimmediatelyafterlensapplication.Thedepositsmaybindsolution
preservatives.Proteindepositionismostcommoninhighwatercontenthydrogels,followedby
lowwatercontentHEMAmaterials.Siliconehydrogelstendtoaccumulateproteintheleast.
Practitionersshoulddirectpatientspronetoheavyproteindepositiontodailydisposables.Ifthis
can'tbedone,recommendanenzymaticcleaneronceeachweekormore.Extendedwear
patientsshouldswitchtodailywear.It'salsohelpfultorefitintoamoreproteindepositresistant
materialsuchassiliconehydrogel.
LipidDeposits
Lipiddepositionhasasmeared,greasywhitishappearance(Figure1).Thelipidarisesfromthe
meibomianglandsofthelids.Patientsmorepronetodepositionincludethosewithtearfilm
potassiumdeficiency,dryeye,highfatdietsandhighalcoholconsumption.Siliconehydrogel
materialstendtoaccumulatemorelipidsthansomeconventionalmaterials.Management
includesthoroughlyrubbingorrinsinglenseswithsolutionusinganalcoholbaseddailycleaner,
whichdissolveslipidsand/orusinglenseswithatwoweekorlessreplacementschedule.
Microorganisms
Thesearegray,black,brownorwhitegrowthsinthelens
matrix.Microorganismsareusuallyfungioryeastand
appearinafilamentarypattern(Figure2).They'recommon
inpatientswhoneglectdailydisinfectionorwhousenon
preservedsaline,highwatercontentlensesand/orlenses
dam
agedordepositedwithprotein.
Lensescontaminatedwithmicroorganismsneedtobe
replaced.Carefulpatientinstruct
ionisnecessaryregardinggoodpersonalhygieneand
properdailydisinfection.It'salsoimportanttoavoid
prolongedlensstoragewithoutdisinfectinglenses.
