Pharm dosing for Module II:

amoxicillin:

30-40mg/kg or 80-90 mg/kg for peds depending on


severity of infection, and adult dosing

acetaminophen:

10-15mg/kg q4-6H pediatric dosing, and adult dosing

ibuprofen:

7.5mg/kg q 6-8H pediatric dosing, and adult dosing

Module III Medications for dosing:


Azithromycin (Zithromax):
Adult dosing:
Azithromycin (Zithromax) 250 or 500mg tablets:
CAP dosage: 500mg once daily on first day followed by 250 mg once daily on
days 2-5 (typical Zpak)
Pediatric dosing:
Azithromycin (Zithromax) 100mg/5ml or 200mg/5mL:
10mg/kg once on first day (max 500mg) followed by 5mg/kg once daily on
days 2-5 (max 250mg)
Ciprofloxacin (Cipro):
Adult dosing: Ciprofloxacin (Cipro) 500mg tabs:
1 tab po every 12 hours for minimum 5 days
Pediatric dosing: rarely indicated
Levofloxacin (Levaquin):
Adult dosing: Levofloxacin (Levaquin) 500mg or 750mg tabs:
CAP dosage: 1 (500mg) tab po once daily for minimum 5 days
HAP dosage: 1 (750 mg) tab po once daily for minimum 7 days (same dose
can be given IV)
Pediatric dosing: rarely indicated
