INCIDENT REPORT

Elisa B. Pizarro , a resident of Sta. Maria, Lal-lo, Cagayan and hired census enumerator of
the Philippine Statistics Authority, met an accident while on her way to her enumeration area,
San Mariano, Lal-lo. The incident happened at about 7:00 AM of August 25, 2015. As a
result, Miss Pizarro sustained bruises on different parts of her body and a fractured clavicle.
She was brought to the Matilde Hospital for medication and was eventually referred to
Puyaoan Clinic for further evaluation of the fracture. Dr. Puyaoan evaluated the fracture and
opted for conservative treatment. He then ordered for the application of a clavicular brace and
prescribed medications accordingly. IPSO Engr. Elena Rivera and HEAD CAS Mrs.
Salvacion Mangoma from the provincial office, were with us and assisted us through out the
day.
