ENROLLMENT AGREEMENT

SOUTHCLIFF WORSHIP FINE ARTS STUDIO

PLEASE read the following information carefully before making a commitment to
study; this form mu st be signed and returned in order for a stu dent to be enrolled.

As

part of this agreement, Southcliff Worship Fine Arts Studio will:

Provide excellence in fine arts training
Begin lessons and classes promptly
Strive for open communication between teachers, parents, and students

As

part of this agreement, we as parents and students will:

Be prepared for lessons and classes
Be prompt in attendance
Follow the make-up policies as outlined in the brochure (one make-up lesson per
semester per student if prior notice is given)
Be prompt in making payments (due on the first day of each month and no later
than the 10th of each month in the semester)
Commit to studying for the entire semester in which the student is enrolled
Participate in end-of-semester recitals as outlined in the brochure

We understand that we are financially responsible for the entire semester

once our student commences study at the Worship Fine Arts Studio.

Drop Policy

If a student chooses to discontinue study, a written notice must be

submitted one month before cessation of lessons. Two months tuition
must be paid - the month of notice and the following month. Any
student dropping after March 15 will be required to pay the remaining
tuition.
