TO INSTRUCTORS TO BE SENT WITH SURVEY



September 15, 2015

Greetings Jeevan,
2016 is looming around the corner, and the CBO has begun planning on how to make next year a success
for all of us. As part of our continued effort to provide quality support to Instructors, we have developed
a self-assessment survey for Instructors to complete.
The purpose behind this questionnaire is to assist us with gaining a better understanding of what
Instructors find important in terms of instruction, management of Center operations, communication
with parents/students, and business analysis.
This survey will be the first step towards identifying priorities and creating a more focused, strategic
2016 development plan. Once you have completed the survey, please save the document using the
same name that it was initially given. Then email the completed survey as an attachment to your field
consultant.
This survey has been designed to be a thorough assessment but should only take you approximately 20
minutes. The deadline for returning the survey is Friday, September 18. This timeframe will allow us to
have sufficient time to schedule a 2016 planning meeting to review the assessment and Center statistics
to develop concrete goals with detailed action steps. Our goal this year is to have the planning meeting
completed by the end of 2015.
The answers to this survey are confidential and will be used for the purpose of working with each
Instructor to identify needs. However, data may also be used anonymously to compile best practices for
the areas of instruction, management of Center operations, communication with parents/students, and
business analysis.
