1. Reference/s: Police Blotter Entry No. 008 Dated July 12, 2016
2. At about 3:55pm, July 12, 2016 a vehicular accident was transpired at
Bayombong Highway, Brgy. Busilac, Bayombong, Nueva Vizcaya involving
Florida Bus bearing plate number TXY 888 and Nissan Van ambulance of
Bayombong RHU bearing plate number of CKJ 834. The driver of Nissan Van
ambulance is known as Allan Pagiit, 40 years old, married and a resident of Don
Domingo Madela, Bayombong, Nueva Vizcaya, however the driver of Florida bus
is known as Alex Pangit, 35 years old, single and resident of Brgy. Don
MarianoMarcos, Bayombong, Nueva Vizcaya.
3. The investigation disclosed that the driver of Florida Bus heading to Manila
had a collision with the Nissan Van heading Tuguegarao due to the loss of break
control while driving.

1.Reference/s:

b. Progress report re; vehicular accident dated July 14,2016
2.On or about 3:55 pm, July 11,2016 a vehicular accident was transpired along Bayombong
Highway, Brgy. Busilac, Bayombong Nueva Vizcaya involving Florida Bus bearing plate number
TXY 888 and Nissan Van ambulance of Bayombong RHU bearing plate number of CKJ 834.
3. As consequence thereof, ambulance incurred heavy damage and the driver of the
ambulance Allan Pagiit sustain a grave injuries that may Couse him in to death. The Florida Bus
driver Alex pangit voluntarily submitted himself to the nearest Police station Bayombong Police
station after the incident while the vehicle involved in the accident were temporarily turnover to
the police station
4.Base on the statement of the witness who where interviewed by the responding police
investigator there was a possibility that the Florida Bus loose its break control resulted to
collision by the Nissan van.
5.Final report will fallow.
