leftundiagnosedanduntreated,theparotitiscancauselifethreateningsepsis.Intheworst
casescenario,theinfectioncandissectintothemediastinumandcausestridorfrom
partialairwayobstruction.Patientswithadvancedparotitiswillhavedysphagiaandsome
respiratoryocclusion.Ifthediagnosisofparotitisisbeingentertained,thepatient
receivesIV,highdose,broadspectrumantibioticswithgoodcoverageof
Staphylococcus,themostcommonagentculturedfromthisdisease.Inthepresenceofa
uctuantarea,incisionplusdrainageisindicated,withcaretakentoavoidthefacialnerve.
Rarely,advanceddiseasemayevenrequireemergencytracheostomy.Mostpatientswith
parotitiswillhavetheconditionarise4to12daysafteraninitialoperation.Becauseof
therapidprogressionofthisdisease,onemustbeawareofthediagnosisand,when
present,instituteimmediatetherapy,includingoccasionalemergencysurgeryforpatients
withanobviousuctuantarea.
