So, Whats a

Clinically Integrated Network?


As many people know, health care
reform changes have continued to
move forward. In response to these challenges,
Gila Regional Medical Center (GRMC) is involved in a
truly collaborative effort with our medical provider community to create a community-wide Clinically Integrated
Network (CIN). A CIN is an organizational structure that
will foster a high degree of partnership toward improving
both the coordination and value of care delivered to our
communities.
Areas of Focus
A specific area of focus for this CIN will be developing
whats called Informational Systems Interoperability. This
will better enable providers to share patient information,
improving care coordination between multiple physicians
and between physicians and the hospital. Another area of

focus will be the development of evidenced-based treatment


protocols to ensure better consistency with proven methods
of medical care. An additional area of focus will involve
working together to create more efficient approaches to
care, in order to minimize overall costs for our patients.
These focus areas, along with many others, will support
the CINs commitment to achieving the Triple Aim of
health care: to improve the experience of care for patients,
decrease the overall cost of care, and improve the health
of our community. In other words, the primary focus of
this big and important effort is the delivery of better,
more cost-effective care to those we serve.
A Strong Network, for Better Care
Another critical benefit of this CIN program will be
increased support for our medical providers, who face new
challenges in being reimbursed for their services. By supporting the success of our local medical practices, along
with a thriving hospital, we can ensure better access to
high-quality care for our communities.
The health care environment continues to change and
present significant challenges to medical providers across
the country, but especially in rural areas. The good news is
that GRMC, our medical community, and other key local
health care agencies are working extremely hard and are
extremely committed to developing and implementing
a best-practice model for the delivery of high-value,
coordinated medical care to you and your loved ones.

New Program Eases Transition

Home After Hospitalization

If you or a loved one has had a hospital stay, you


know that transitioning home isnt always easy.

Avocado Green
Onion Party Dip
This quick, delicious dip also
provides vitamins A and C.
Ingredients:
medium avocado, peeled and
diced
1 cup low fat cottage cheese
cup plain, nonfat yogurt
cup sliced green onions
cup shredded carrots
1 Tbsp. fresh lemon juice
cup nonfat mayonnaise
2 cups broccoli florets
1 cup cucumber slices
28 melba toast rounds
Directions:
Toss avocado with lemon juice;
set aside. In food processor or
blender, blend cottage cheese,
yogurt, and mayonnaise until
smooth. Add cottage cheese mixture to avocado; stir in onions and
carrots. Cover and chill. Serve with
vegetables and melba toast.

But having a support system can help you better follow your treatment plan,
navigate follow-up care, and take charge of your health. With this in mind,
GRMC recently introduced its new Lay Caregiver Program.
The Lay Caregiver Program allows patients to name someonesuch as a
family member or friendas their caregiver. If the person is willing, we include
them in the patients care and give them the tools they need to support the
patient at home, explains Peggy White, R.N., Chief Nursing Officer for GRMC.
Involving the Caregiver
Nurses or patient case managers can work with caregivers in different ways.
Caregivers may sit in on discharge instructions to understand what medications
the patient should take and when to seek follow-up care. They can also learn
how to assess the patient at home and watch for complications. If appropriate,
caregivers may learn to change the patients wound dressings.
The program is tailored to each patients needs, says Jan Zolotar, Director
of Case Management for GRMC. Every person is different.
Improving Patient Outcomes
Although most patients admitted to GRMC will be asked if they would like to
name a caregiver, they arent required to do so. And the person named does not
need to accept the role if he or she doesnt feel comfortable.
This program is completely voluntary, explains White. However, for those
who participate, weve already seen positive results.
We believe in encouraging people to take more accountability for each
other, adds Zolotar. Being a caregiver doesnt have to be overwhelming. But
providing that extra layer of support at home can make a big difference.

BCWC offers membership levels
that will fit the needs of anyone. A
variety of group fitness and pool
classes, as well as open swim hours,
are offered year-round. In the new
year, BCWC will be hosting a January
Weight-Loss Challenge, and membership specials will be offered in the
early spring.
