The document is a nursing skills checklist from Bon Secours Memorial College of Nursing. It lists the student's name and instructors for two nursing courses. It instructs students to indicate the date and whether a skill was performed in lab (L) or with a patient (P). Students should update the form if a previously lab-practiced skill is later performed with a patient. Skills from the first course don't need to be re-recorded in the second course. Students must keep the checklist up-to-date in their portfolio and submit it at midterm and final to their clinical instructor.
