A SWMS REVIEW CHECKLIST


Note: Each subcontractor, before commencing work, must provide written Safe Work Method Statement/s (SWMS) for the work to be carried out. The following checklist
is to be completed and reviewed by PIC, and assessed to have met the COMPANY acceptance criteria in compliance with the requirements outlined in the checklist and
any relevant EH&S Legislation, Codes and Specifications.

SWMS controls.
14. Identifies where any Subcontractors will be used, who is responsible to monitor and supervise the
works, and how hazards arising from interaction between trades will be communicated and
managed (e.g. induction into more than 1 SWMS).
15. For complex or high risk work activities (where appropriate), uses photos / diagrams / sketches /
summary to enhance the SWMS.
16. Includes the name and signature of the person who developed the SWMS, a current date and
revision number.
17. The SWMS includes specific EH&S controls for key high risk hazards as listed below:

Piling rig assembly
Rig/ crane mobilisation at site
Components mobilisation at site will be placed where works will start.
Assembly of pile rig with leader extension first
Installation of pin, bolts to secure the two components.
Leaders will be raised by pile rig vertically, once position is secured upright hammer will be stationed to position near
the rig ease to connect the cables and wiring.
Hammer installation is secured the pile rig will pull the hammer up just few feet from the ground.
Installation of bolts to secure the hammer to the leader will be carried out. Cables will be installed to the power pack to
connect the hammer.
Test the hammer is secured by hoisting upwards a few feet from the stationery position.
Installation completed. Housekeeping works will be carried out.
