LIST OF MANDATORY DOCUMENTS TO BE SUBMITTED ALONG WITH BID ::

i. Details of Payment for Tender Schedule Cost ( Rs.525/- ) for Each Zone
ii. Details of Payment for EMD Rs.30,000/- ( Rupees Thirty Thousand Only )
iii. Attested Copy of current NSIC certificate for exemption from EMD.
iv. Tender documents in original, duly filled in and signed by the Tenderer on all pages.
If the Tenderer fails to sign in all the pages of Tender Document, their Bid will be
rejected.
v. Letter of authorization if any.
vi. Attested Copy of Experience Certificate Optical Fibre Cable laying experience certificate
issued by an officer not below the rank of Divisional Engineer ( BSNL ), in-charge of the
execution, certifying the quality. The labourers to be engaged should be well versed in Local
Language. The experience certificate should be in the name of Company / Firm and
not in the name of the Partner / Employees & the same may be produced in Original
when required by BSNL.
vii. Attested Copy of Certificate of Registration of firms in case of Firm / Agency from the
appropriate authority.
viii. Attested Copy of Partnership Deed in case of partnership Firm / Agency.
ix. Attested Copy of authorization of the competent authority for signing the Tender /
Document / Bid / Agreement Etc.,
x. Attested Copy of Proof of Turn over of Rs.10,00,000/- ( Rupees Ten Lakhs ) or more
per annum in last 2 years and duly attested by State / Central Govt. Officers.
xi. Attested Copy of ESI Registration Certificate and Latest Payment Certificate from ESI
Corporation.
xii. Attested Copy of Employees Provident Fund Registration Certificate & Proof and Latest
Payment Certificate from EPF Department.
xiii. The successful tenderer should produce Solvency certificate for Rs.3,00,000/- [ Rupees
Three Lakhs ] obtained from Tahsildar or from the banker of the tenderer which shall
not be older than the date of issue of NIT.
xiv. Attested Copy Service Tax registration Certificate details.
xv. Attested Copy Labour License issued by appropriate Labour Authority ( Central )
xvi. Attested Copy of Registration of Commercial Tax / Sales Tax Certificate if any.
xvii. Attested Copy PAN Card / Latest Income Tax Clearance Certificate for the Last Two
Years.
xviii. Declaration to the effect that no near relative is working in BSNL in the enclosed
format.
xix. Declaration for Non Black Listing of the Firm / Agency / Company.
xx. An affidavit as available in Section - 13
xxi. WAX / Tape Sealed Financial Bid in a separate Envelope( 3rd Envelope ).

Note ::
a. Attestation shall be done by any Gazetted Officers or by any Executive of PSUs / BSNL not less
than AGMs in BSNL./Self attestation.
b. Non Submission of any or all the above said documents will result in the rejection of Bid.
