Spot report on reckless imprudence resulting to physical injuries and damage

of property.
That on or about 3:30 oclock in the afternoon of April 16, 2013 along the
National Highway located at Sitio Lag-itan, Brgy. Doljo, Panglao, Bohol. Initial
investigation was conducted by the elements of this station revealed that on above-
mentioned time, place and date, while Honda XRM 125cc black and white
combination bearing plate no. 475IO registered under the name of Gina Matig-a,
driven by one Melchor Matig-a y Alcala, 43 years old, married, a habal-habal driver
and a residence of Brgy. Tangnan, Panglao, Bohol holder of Non-professional drivers
license no. GO3-11-002306 expires on 2014-01-07, with back rider Mary Clenista, of
Brgy. Doljo, Panglao, Bohol was running heading towards Brgy. Doljo this municipality
accidentally bumped to the left side mirror of Isuzu Canter Drop side truck, color
green bearing plate no. GTR201, registered under the name of Marvin Bajao,
Tutolan, Dauis, Bohol, driven byDanilo Epanag y Galindis, 36 years old, married,
Delivery truck driver and a residence of Cayam, Garcia Hernandez, Bohol not carried
driver license, traversing in opposite direction (from Brgy. Doljo headed to Brgy.
Poblacion this municipality). The driver and the back rider sustained injuries at
different parts of body and were rushed to a hospital in Tagbilaran City for medical
treatment while the driver of delivery truck and said motorcycle were brought to this
office for documentation and, for proper disposition. The driver and the back rider of
said motorcycle were not wearing crash helmet. Any development, progress reports
will follow.
