    Two water leaks were observed in the LP evaporator's third header on both the east and west sides during an inspection on October 15, 2015. Four tubes total were found to be leaking through visual inspection and after cutting access plates. The leaking tubes were plugged to stop the leaks and a leak test found no further issues after repair.

    Leakage observed at West segment of LP evaporators 3 rd [Last] Header Both Side

    Work details:

    At first Visual inspection done on 15 Oct, 2015


    Leakage observed at both East and West segment of LP evaporators Last header [3 rd Header]
    Both side
    After cutting patch plate on that side, visually two tube leakage observed on both east and west
    side of that header

    Action has been taken:

    All 4 tubes has been made blind


    Leak test has been performed after tube plugged. No leakage observed
    LP Evaporator West Header West Side: 2 Tubes Blocked

    West Side [Patch Plate Cutting] West Side [Tube Plugged]

    LP Evaporator West Header East Side: 2 Tubes Blocked

    LPEV-3 East Side [Plugging] East Side [Patch Plate Re-Installed]
