I am a Medical Doctor specializing in the field of Internal Medicine. I have
been a registered Medical Practitioner in Belize since 2001. On todays date, the 13th day of May 2015, I examined a patient by the name of Sarah Jessica Parker, Justice of the Peace of 12 Purse Strings Lane, D.O.B 12.03.55 who was admitted to the ward at the Belize Private Hospital. My findings are as follows: Patient was in acute respiratory distress at time of examination resulting from complications due to history of Type 2 Diabetes.
Due to the medical condition of patient continued hospitalization for an
indeterminate period of time is required. Based on results of my examination I find that the patients condition is critical and that she is too ill to travel out of the hospital.
