Heres how:
1) Submit a registration form (required EVERY year)
2) Maintain 95% attendance (no more than 2
absences)
3) Avoid any Ds and Fs
4) Submit a copy of your grades to JRJF EVERY
quarter
 High School 2016-2017 Program Guidelines
Program Details and Guidelines:

Students will be awarded $50 scholarships for every A and $25 scholarships for every B earned per
quarter in subjects within following categories: humanities, mathematics, science, and history.
All awards will be deposited into an interest-bearing 529 College Savings Account.
Funds earned are solely for higher education and military academies and will be accessible to the
student upon graduation from high school and enrollment in a program of higher education.

In order to be an active member of the A-Team Scholars Program, students will follow the
guidelines below:
Enrollment in a high school in Chicago, IL
Submit completed registration form to the James R. Jordan Foundation EVERY year throughout high
school
Submit copy of report card to the James R. Jordan Foundation EVERY quarter
Maintain 95% attendance throughout the quarter
Avoid grades of D or F, as these will cancel any money earned for As & Bs.
Update the James R. Jordan Foundation with any changes in address and/or phone number (contact
information on other side).
 A-Team Scholars Parent & Guardian Expectations

As the parent or guardian of an A-Team Scholar, you play perhaps the most important
role in ensuring your student achieves to their highest potential. We may provide the
scholarship, but you provide the love, support, and encouragement they need to
succeed.

As an A-Team Parent or Guardian, I Pledge:

To do what is necessary to ensure my child excels academically.

To do what is necessary to make sure my child arrives to school on time and in
uniform.
To ensure my childs report card is submitted to the James R. Jordan Foundation
EVERY quarter while they are in high school.
To continually talk to my child about the importance of continuing his/her
education after high school graduation.
To assist my child in continuing his/her education after high school.
To seek the help my student needs when his/her needs go beyond my own
knowledge, experience, and skills.
To update the James R. Jordan Foundation with any address or phone number
changes throughout enrollment in the A-Team Scholars Program.
To be first and foremost a source of love and support for my child.
