Agreement of Donation of oxygen concentrators

(Philips EverFlo Home oxygen system)

The agreement is made in date ________ between donor Sumit Verma and Old age
home Madhur-Bhav Homes at Pune.
The donation is made as sole purpose of charity. Where donor Sumit Verma
donating his 6 month used Philips EverFlo Home oxygen system which is having 3
years limited warranty from Philips.
The donation is made based on the following term and condition stated below.

Sumit Verma will hand over Philips EverFlo Home oxygen system on as is
condition (working functional) along with ownership to the device to
MadhurBhav Home and now no longer will be owner of device/machine.

Madhur- Bhav Home understand all the technical use and process to operate
the machine.

Madhur- Bhav Home understand how to do regular maintenance /repair of the

Machine (Philips EverFlo Home oxygen system) and will not involve Donor
Sumit Verma for any of such activity. The cost of future maintenance will be
borne by Madhur- Bhav Home

Madhur Bhav home will use the Machine (Philips EverFlo Home oxygen
system) for helping old age people residing in their home and will not use the
device for any commercial use.

Madhur Bhav home dose not own the right to sell the device (Philips EverFlo
Home oxygen system) outside old age home.

Sumit Verma can come on regular check to Madhur Bhav home to check if
the Machine(Philips EverFlo Home oxygen system) is been used
