Leaking of Urination in the Mens Toilet Room:-

Background.

We all know that In March 2014, an over-pressurisation within the Unit 3 boiler rendered the unit inoperable. The
over-pressurisation resulted in substantial damage to the boiler itself including the surrounding associated
structural components. It has been determined that the existing boiler and associated damaged infrastructure is
to be demolished and replaced accordingly by the Main Boiler Contractor.

The project recovery team appointed to handle all works associated with the re-instatement of Duvha Unit 3 has
designated working spaces for the Main Boiler Contractor and their subsequent activities. Area D has been
designated as a storage region for all demolished metals from the boiler.

Cause.

At Mens Closet room the floor was observed wet. It was mainly during early morning hours. After a thorough
investigation understand that a leak was observed during Urination, without wasting time self had informed to
SHE Rap for putting defect notification as it was not hygienic. Annexure -3, attachment mail regarding defect
Notifications. Also for quick response self had given a call to Mrs. Thembka Z for confirming the mail had been
acknowledged from her side. Following to that - next day self had given a call to Mrs. Thembka find whether
defect notifications had been raised and feedback from the respective maintenance dept. for their representative
to attend the defect at the earliest.

Feedback.

Dated on 27th Feb 2017, respective Maintenance Dept. personnel had visited the Recovery office for the
inspection. Self was assigned by Mrs. Thembka to show the defect. Self accompany them and shown the defect
and they understand the proper reason behind the same. They told that will get back to us within a day.

Conclusion.

The Personnel from Maintenance Dept. had arrived to site and rectify the defect within two hours with proper
PPE and safe working acts. For satisfactorily performance, self-had went thorough and a quick check to know
the defect had been closed or not.

Moral:

Prevention is better than Cure, this is a famous proverb, but sometimes systems and equipments fail due to
many issues, so Quick reaction with effective action will sometimes manage to control the damage beyond the
intensive manner.
