Artifact Title: Student Behavior Form (CICO)

of the school districts student achievement goals.

Description of the artifact and how it meets the standard and criteria:

The artifact that I chose to include to show my strength and competency in this standard is a
behavior form that was created to encourage a student to improve behavior. At the end of each
period the instructor scans a QR code that bring up the google form. The teacher is to rate the
student for their behavior that day. This form is strictly for the monitoring of behavior.

Reflection on the artifact:

This form is used to improve a students behavior inside the classroom. The student can earn
points each day that he has a score higher than 75%. The student can then use his accumulated
points to buy certain items, or privileges. We are seeing that as the behavior improves for the
student, so do his academics. It has been a really exciting process to watch the student change
inside the classroom because of the adult support he now has, and the carrot that we have put out
there for him.
