The member had several significant and immediate needs.

Just recently, she was diagnosed as diabetic. However, she wasnt
aware of any nutritional counseling and did not have any glucose
monitoring supplies. I called her primary care provider to help
arrange nutritional classes and needed supplies.
More concerning, the member had been experiencing post-
menopausal bleeding for the past year. This is a high risk for
cancer and calls for timely evaluation.
Due to lack of transportation, the member was unable to see the
specialist referred to her by her doctor. I contacted my own
OB/GYN physician, who agreed to see the member as soon as
possible. The member said she would be able to make
arrangements with her granddaughter for this appointment
although I told her to call me if that didnt work out and I would
take her.
To help ensure she got the help she needed, I got a warm transfer
to the health plans care management team and addressed each
of her needs.
NOTE: Following Faiths visit, the member contacted the Matrix
Member Feedback Line. She was in tears as she expressed how
much she was touched by Faiths heart and spirit.
