    AUSTRALIAN VISA APPLICATION

    INSTRUCTION:

    Important Please read these instructions carefully in completing your application.

    1. Click here to open the Application form and provide information needed. For
    your reference, you may download this Sample Form

    Reminder: Once you complete your application, we strongly advise that you review the
    information you entered for accuracy prior printing

    2. CREW shall have the below upon reporting to Fleet:


    a. POEA Contract in 4 copies in legal/ long bond paper Click here
    b. Photocopy of Valid Seamans Book (note seamans book shall be valid at least
    8 months prior joining date)
    c. Photocopy of Valid Passport (note passport shall be valid at least 8 months prior
    joining date)
    d. Maritime Crew Visa (MCV) Please click here on how to apply for MCV
    e. Completely Filled up application form
    f. Two (2) recent photo (Passport size)
    2. Visa fee for walk-in (LOG service / Logistic Fee Php1000.00) cash
