581 Crew Prerequisite Form
581 Crew Prerequisite Form
581 projects need to have all key crew positions producer(s), cinematographer, editor, production and post
production sound approved by Student Affairs AT LEAST THREE WEEKS BEFORE BEGINNING SHOOTING. Projects can
have up to three producers but only one is required. Please note that you must meet with Rich Hyland, Sound
Department Manager, and get his approval on sound personnel, before taking this form to Student Affairs for approval.
This is a separate meeting from the one you must have later with Rich Hyland to obtain Greenlight approval.
Once you have obtained the approval of Student Affairs, you must present this form along with the Green Light To Shoot
Signoff Form at all your Green Light meetings. THE ENTIRE GREEN LIGHT PROCESS MUST BE COMPLETED AT LEAST TWO
WEEKS PRIOR TO SHOOTING.
Exceptions to the requirements below are sometimes possible, depending on such factors as the crew position, whether
or not the crew member is enrolled in the course, if they have extensive practical experience (e.g. in the industry or at
another film school), if school equipment will be used, and if there is more than one crew member in that position. Any
and all exceptions to the above requirements must be approved in advance in writing by the faculty advisor, the 581
Coordinator and in some cases by the head of the track. Please get all SPECIAL APPROVAL SIGNATURES and FILL OUT
THIS FORM COMPLETELY before taking it to Student Affairs.
