AFFIDAVIT OF DISCREPANCY

I, XXXXXXXXXX, Filipino, of legal age, single, and a resident of

xxxxxxxxx, after having been duly sworn to in accordance with law, do hereby
depose and state:

1. My mothers full name after her marriage with my father is xxxxxxxxxxxx.

Attached herewith is my mothers Certificate of Birth and Marriage

Contract.

2. Thus, my full name at birth was supposed to be xxxxxxxxxxxx;

3. However, due to typographical error by the local civil registrar, my name

was registered as xxxxxxxxxxxx, with my middle name misspelled as such.

Attached herewith is my Certificate of Live Birth.

4. I have customarily used both in my school and employment records the

correct spelling of my name which is xxxxxxxxxxxx, and not
xxxxxxxxxxxx.

5. I first discovered the discrepancy between the spelling of my middle name

as appearing in my Certificate of Live Birth and my true and correct middle
name only in 2005 and I did not take the time to correct the same because
there has been no need for me to do so since.

6. I recently claimed for the death benefits due from the International Wiring
Systems (Phils.) Corp owing to me by reason of my mothers death, one of
the requirements for which is my Certificate of Live Birth.

7. I am executing this affidavit to attest to the truth of the foregoing, for the
information of all proper authorities concerned, and for whatever legal
intents and purpose it may serve.
