AFFIDAVIT

I, NEIL S. ROLDAN, of legal age, single and residing at No. 128 Doce
36 St. East Bagong Barrio, Caloocan City, after having been duly sworn to in
accordance with law, hereby depose and state:

1. That I am a holder of a Philippine Drivers License with License

No. N2512015085 issued on 4 April 2015;

2. That sometime in May 2015, I lost my wallet and the said

drivers license was inside the lost wallet;

3. That I took great pains to look for the said wallet with my
drivers license in it but to no avail;

4. That I am issuing this affidavit in all good faith to attest to the

veracity of the foregoing facts.

AFFIANT FURTHER SAYETH NAUGHT.
