After my visit with the husband and wife, I followed up with a call
to their daughter. She had a number of concerns about her
parents, but didnt know where to turn. She asked if I thought
there was enough information from my visit for an Adult
Protective Services (APS) referral. I told her I had contacted their
primary care provider and made a care management referral, and
due to her concerns, I would also place an APS referral.
She then asked if I would have a conference call with her brothers
to share my findings, which we did the following day. I shared my
concerns and provided information on available resources. We
discussed power of attorney paperwork and possibly assisted
living, home health, physical and occupational therapy and how
to obtain these services. I also provided them with Meals on
Wheels, Area Agency on Aging and respite/adult daycare
information.
The family was very thankful for my time, sharing my concerns
and providing the information. The daughter said she would
contact the doctor to obtain orders for services and would also
follow up on the community resources.
